What is a fertility check?

Female and male fertility depend on age, genetic factors, lifestyle, and the environment around us. Most people wish to start a family and have children at some point in their lives. To stay informed about your fertility, we recommend that women visit a gynaecologist regularly. A semen analysis provides a quick overview of male fertility.

Female fertility

A woman’s fertility primarily depends on age, hormonal balance, ovarian reserve, and the health of the uterus and fallopian tubes. The most important factor affecting female fertility is age – fertility begins to gradually decline after the age of 30 and decreases more rapidly after 35.

In addition to age, fertility is also influenced by lifestyle, stress, body weight, smoking, and various health conditions such as endometriosis, polycystic ovary syndrome (PCOS), or thyroid disorders. Often, fertility-related issues may not present clear symptoms, meaning a woman may feel completely healthy, yet conception may still be unsuccessful.

Early awareness of one’s fertility provides the opportunity to make informed decisions – for example, not postponing family planning or considering egg freezing.

A female fertility check includes a thorough consultation with a gynecologist and a gynecological examination. With the help of ultrasound and laboratory tests, it is possible to assess ovarian reserve and overall reproductive health. If pregnancy has not occurred for any reason, the doctor can help identify possible causes of infertility and recommend further tests and treatment based on the individual situation.

    In about half of infertility cases, the cause is male-related

    Male fertility

    Male fertility primarily depends on sperm count, motility, and quality, as well as overall health and lifestyle. Although fertility is often thought of as mainly a female issue, about half of conception difficulties are due to male factors.

    Sperm quality is affected by age, excess weight, smoking, excessive alcohol consumption, stress, lack of sleep, and exposure to heat or environmental toxins. Hormonal imbalances, previous infections, or chronic illnesses can also play a role. In many cases, there are no clear symptoms, and a man may feel completely healthy while sperm quality is reduced.

    The primary test for male fertility is a semen analysis, which evaluates sperm count, motility, and morphology. If abnormalities are found in these parameters, it is recommended to consult a male fertility specialist (andrologist), who may suggest additional tests and examinations if needed.

      It’s advisable to consult an infertility specialist if you haven’t been able to conceive after 1 year of regular, unprotected sexual intercourse. From the age of 35, it’s recommended to start infertility treatment procedures after just 6 months.

      Before starting infertility treatment, both partners undergo thorough examinations to find out the possible reasons why conception has not been successful so far. This usually includes:

      For women:

      • Hormonal blood tests (e.g., AMH, FSH, LH, estradiol)

      • Ultrasound examination to assess ovaries and uterus

      • If necessary, fallopian tube patency check (e.g., HSG or HyCoSy)

      • Gynecological examination

      • Tests for sexually transmitted diseases (e.g., chlamydia)

      For men:

      • Semen analysis

      • Blood tests (hormonal and STI tests, if necessary)

      Sometimes, genetic studies may also be added depending on the individual situation.

      The duration of infertility treatment depends on the chosen treatment method and individual response to treatment. For example:

      • IVF (in vitro fertilization): From the initial consultation to embryo transfer, it takes approximately 4–6 weeks.

      • IUI (intrauterine insemination): Can occur within 1 month, depending on the cycle.

      • If donor cells are used or additional tests are performed, the process might be slightly longer.

      If the first attempt isn’t successful, the next cycle can start within a few months.

      Infertility treatment isn’t completely free in Estonia, but in certain cases, it’s supported by the Health Insurance Fund.

      If a woman has been diagnosed with infertility and is health-insured and up to 40 years old (inclusive), the Health Insurance Fund covers infertility treatment costs—including doctor’s visits, procedures, and pregnancy detection.

      Important to know:
      The initial consultation and tests to determine the cause of infertility are chargeable for all patients, even those with Health Insurance Fund coverage.
